Alternativa metoder inom psykvården

Jag tycker det är tragiskt att det ifinns så mycket destruktiva metoder vad gäller det mentala. Deprimerade människor får ström genom huvudet och oroliga barn ges psykofarmaka.Det verkar som om kemikalier ska lösa alla problem

Blir man frisk av att få 200 volt genom sin hjärna?? Det har jag väldigt svårt att tro.Däremot vet jag att det finns biverkningar som knappast är att leka med. Psykkofarmaka kan man bli beroende utav och kemikalier kommer aldrig att vara en lösning på mentala/personliga problem.

Är det inte dags för alternativa metoder inom psykvården?

Visst kan det vara det, men det är lättare sagt än gjort.

Har själv läst en del i ämnet och det finns vissa debattartiklar som tar upp det, både för och emot t.ex psykmedicinering.

en debattartikel i GP från 2003-03-02 "Psykmedicin inte av ondo" motsäger detta (har inte tillgång till den digitalt).

Går ifrån dina metoder med ström i huvudet, men ändå ett intressant tillägg om att allt ska kunna lösas med kemikaler som du påpekar.

Nja det med stömmen är nog inte så farligt som det låter. Det är förknippat med tvångsomhändertagningar och bahandlingar.
I dagsläget är det många som efterfrågar det och jag har två personliga referenser som har positiva erfarenheter av det.

Det funkar lite som en reboot för signalsystemet har jag fått förklarat för mig. Av den ena som var positiv till det och jobba inom sjukvård.

Psykofarmika däremot är ganska bissart, såg ett program om ett läkemedelsföretag som hittade en substans som hade effekt och efter det skapade ett symptom :)

Fö är dom ju inte ens säkra på hur antidepressiva fungerar.
Får nog fan skriva en tråd om den sviniga läkemedelsindustrin.

Vilka alternativa metoder tänkte du dig? Visst kan man bli beroende av psykofarmaka, men är det så farligt då? Hela livet går nog för merparten av människorna ut på att minimera smärta. Tycker faktiskt att det är lite överförmynderi att människor som har grav ångestproblematik mm många gånger nekas lindring i form av stesolid och liknande preparat.

Alternativa metoder finns och provas alltid i första hand innan tex ECT
(elbehandling) används vid depressionstillstånd.
Jämför med den somatiska vården; om en person har för höga blodfett-värden så provas först kostomläggning, motion mm. Fungerar inte detta får man istället mediciner som sänker blodfetterna.
Samma sak gäller depressioner; här försöker man oftast med samtalsterapi, kognitiv terapi och/eller medicinering häva depressionen.
Ibland är depressionen så grav och handikappande för patienten att detta inte hjälper. Eller om risk för självmord finns. Då finns ETC att tillgå och det är en smärtfri behandling som är relativt fri från biverkningar.
Kärlek och respekt, ska det vara så jävla svårt?

Problemet ligger nog i det att man har för kite tid per pacient och därför använder man sanbba lösningar. Själv tycker jag att det är fäll. Man ska se vailken lösning som är bäst för enstaka situation och sedan bästemma om det bli terapi eller medicinering

Här är en kortare uppsats jag skrev om ämnet när jag gick i skolan:


Listen to Prozac?


The use of Antidepressants, or more specifically so called SSRI’s (Selective serotonin reuptake inhibitors), has been highly increased during the last ten years; and while the medical community supports the viewpoint that antidepressant are the major reason for a decline in suicide-rates, Örjan Ericsson on the National Board for Health and Welfare has a different explanation:

“Even though the decline increased more when they (the SSRI’s) where introduced I think that the reduction would have to be stronger to draw the conclusion that SSRI prescriptions explains the trend. It is foremost among men that suicides have decreased, while women are the big consumers of SSRI’s”

Another aspect that makes Örjan Ericsson question the SSRI’s as an explanation, is that the actual suicide-attempts haven’t decreased:

“They should be a better measurement on how people actually feel and thus one should get a better effect on them from medical treatment. But I see nothing in the statistics that could be interpreted as antidepressants having a great effect”

So do antidepressants help? While there has been a, although lukewarm – debate regarding the use of antidepressants in the medical community and where especially therapists and psychiatrists have opposed this “robotification” of man; the public has none, or little, knowledge on how SRRI’s actually work. The Selective serotonin reuptake inhibitors derive from research (by the noble prize winner Dr. Axelrod for one ) that showed lower levels of serotonin in the brain among depressed. The brain is made up of neurons, and those neurons are basically interconnected brain cells. “Messages” in form of impulses travel along these cells, and when a message reaches the end of a neuron, it has to jump a gap, called a synapse, in order to get to the next neuron. In order to do this, the neuron releases small amounts of a chemical - called a neurotransmitter – into the gap between the nerve cells. What usually happens is that the body takes the neurotransmitter back into the original nerve (neuron) so that is has a renewed level of its neurotransmitter(s), making it able to send another “message”, or impulse.
What research has shown is that during a depression, certain neurotransmitters such as serotonin are reduced. SSRI’s slow down the process of returning serotonin to the neuron, and this makes it more likely that enough serotonin will build up to set off the impulse to the next neuron. SSRI’s are thus not really raising the levels of serotonin, but rather enables a more efficient use.
   Even if the drug Soma, from Aldous Huxley’s Brave New World, often has been used as an example, SSRI’s are no “happy pills”. Unlike certain Americans who in the famous book Listen to Prozac changes their lifestyle completely and turn into party-freaks, there is no “happy pill”, drug like effect in general. Placebo being part of the picture, it is not hard to imagine how culture comes into context with the reactions to SSRI’s, and thus the average Swedish “patients” have reacted less dramatic. However personality changes is a reported side effect of SSRI’s, and thus many, among them the professor of psychiatry at the Karolinska institute Lars Farde  - has recommended not to give these preparations to adolescents who “still” are developing themselves in many ways (“there is not much known about the context – the brain, the nerve fibres, neurotransmitters etc. – in which the medicine works” ). And yet, the prescriptions for this group are increasing rapidly4. Many from the medical community are “pro Prozac”, including Björn Beerman, professor in medicine and head of the Medical Products Agency who has been quoted in the media for wanting to increase the prescription of SSRI’s. In an article by the Swedish Daily he replies:

- Yes, there is a great suffering for the patient and the risk of suicide is overwhelming among those who are depressed. Maybe that could be avoided in a higher extent if more people were on prescription, both from antidepressants and medicine in general”  

What to make of this medication-frenzy? That despite a cost of around 1,1 billion Swedish kronor for antidepressants and 22,5 billion Swedish kronor for prescript medicine in general during 2005 , the majority in the medical community wants to increase prescriptions. Now to understand this, one must look upon the chemical philosophy of today’s psychiatry; as natural sciences have taken monopoly of the word truth and reality during the 20th century, so has the religious and spiritual world-view haven to give in for the measurable. The debate has swayed back and forth, but the general picture is still that of biology kicking Freud out of the picture.
The idea (knowledge) that our biology affects our psychology is really nothing new - Plato first suggested that the brain is the mechanism of mental processes  - however more and more emphasis has been put on the genetic, rather than the bio-social (and unconscious) perspective lately. That humans are highly social animals and are very much affected by our surroundings – both our social and natural environment – has not always been taken into account by the medical community; especially not in Sweden.
   So what is the cause of depression? Is it the serotonin-levels themselves - something being wrong in the hard-wiring of the brain - or is it social circumstances that cause depressions and lower serotonin-levels; what comes first, the hen or the egg? The answer has yet to be found in fact (“We have identified the foundational stones,  but are yet to find their functions” ), but it is not hard to imagine a spiral in which our environment (social and natural) cause melancholy, resulting in lower levels of serotonin, which in turn results in more melancholy etc. Speaking as someone who himself have been depressed, the hardest thing in getting rid of a depression is that one has to work actively against the “whirlwind of words” in ones head. It only gets worse before it gets better, and as the whirlwind tries to suck you back in you have to keep struggling to walk away.
   Freud would say that the depression is caused by unconscious guilt caused by aggressive impulses towards an inner object. He described depression as being caused by the loss of an object of high value for the ego. The loss of this object, which is loaded ambivalently (love – aggression) - leads to critic and accusations, and if this is connected with an undeveloped and/or fragile ego-structure (the ego and object are one), this critic turns inward. This is because the ego is so dependant of this object, that the loss is intolerable both for ego and in turn the superego which can not accept the dependence. Thus the superego turns its aggression towards the ego (object). And as the superego is made up both by “judge” and “conscience”, the conscience manifests itself in unconscious guilt; being acted out as self-reproaches and attacks on the ego.

Freud never really talked about how the brain influences the mind, and thus really has nothing to say either for or against antidepressants. In the beginning he, as a physician (and influenced by the general view that natural science was going to explain everything) started from a fairly natural scientific perspective, talking about hormones and hoping that natural science in time would be able to explain psychology. However he left natural science pretty soon, and rather approached the mind from a hermeneutic point-of-view . This has been criticised as being “impossible to prove” and a “pseudo-science” by the medical community . The fact that Empiricism more and more has become the only approved scientific method has not only marginalized psychoanalysis as a way of treatment, but also has other sciences who don’t deal with Empirics. This derives from a monistic world-view, in which “external (objective) reality” rather then correspondent inner (subjective) reality is the point-of-interest. However, even having a monistic world approach; can one really disregard our environment? Even if one is to say that the mind only is a composition in which our biology is the conductor, and that the soul is nonexistent, are genetics the only matter of interest? Many in the medical community of Sweden seem to think that, and so pills are considered the only needed way of treating not only depression, but all kinds of mental illness.
   According to psychotherapist Eva (efternamn borttaget), some psychiatric open-wards in Stockholm demanded patients to - via a general practitioner - try antidepressants (often Cipramil) before they even could be offered consultation . The politicians want a “quick-fix” to a minimum cost, and since community has taken so much of the financial responsibility from the individual (the Swedish “folkhemmet”), there is an inclination towards theories which are compatible with the budget. Since pills are cheaper than therapy, at least in the short-term perspective – this is what’s been given. You can get SSRI’s by phone, but you will not be offered therapy; the answer is that you will have to go to private alternatives after a few sessions of cognitive “therapy” (how can 3 sessions be considered therapy?). But antidepressants can only remove symptoms and thus possibly enable change, not actually cure a problem caused by loss of object. Antidepressants and therapy are not opposites, but can effectively be combined, so why isn’t this done more frequently? The answer is again economical, philosophical and political.

ECT

Det finns många berättelser på internet från personer som fått ECT och har upplevt den "negativa sidan". En del pratar om att de fått hjärnskador av ECT. Att minnet blivit raderat o de kommer ej ihåg personer. Låter ju hemskt.

Jag har fått ECT. Jag har lite minnesluckor om vad som hände i den vevan, men annars har jag inte försämrat kortids- eller långtidsminne. ECTn fick bukt på min depression och det var ju syftet. Jag tror att man ofta bildar sig sig en skräckscenariobild a´la Gökboet, men så fungerar det inte. Det är en smärtfri och effektiv behandling. Att jag har lite dimma över vad som hände i den vevan tycker jag inte är ett så farligt pris.

Fick jag minsann också ETC när jag skulle testa om mitt lilla 1,5v batteri till mp3 skulle funka. Körde tungan rätt i och fick en smärre choch när jag såg prislappen på batteriet.

Är inte detta ett skitsnack?

Anthony,

Det vore bättre om du istället för att ta del av lösryckta berättelser på Internet skulle ange en seriös källa som gav stöd för att det krävs alternativ vård av psykiskt sjuka.

Hur skulle denna alternativa vård till den vetenskapligt baserade vården se ut enligt ditt förmenande?

Det gick att tolka på olika sätt. Jag förstod inte om du var ironisk eller om du höll med mig om att det var diffust med vad "alternativ vård" var för något.

Du kommer säkert bli förvånad av att höra det här, men andra människor har redan känt den känslan du ger uttryck för här, och gissa vad? Det har gjorts utvärderingar av effektiviteten hos ECT! Så din tråd var helt onödig. Fakta finns redan, så varför spekulera och ge uttryck för en massa spekulationer, fördomar och irrationalitet?

Däremot vet jag att det finns biverkningar som knappast är att leka med. Psykkofarmaka kan man bli beroende utav


Visst, och verktyg kan man skada sig på.  :roll:  Är detta ett argument till att hantverkare bör avstå från verktyg?

och kemikalier kommer aldrig att vara en lösning på mentala/personliga problem.


Hade du provat "kemikalier" hade du sannolikt aldrig skrivit det där dumma inlägget. Sannolikt det mest ointelligenta jag läst på Filosofiforum sen jag kom hit.
